RE: Columbus Shoulder Surgery Relocation
Dear Patient,
To begin, I would like to thank you for the trust you have given me as your Orthopaedic Surgeon. It has been an honor and a privilege treating you and/or your family members and having you as a part of my practice. Your continued health and wellness is a priority for me.
After long and thoughtful consideration, I am relocating my practice, Columbus Shoulder Surgery & Sports Medicine to Largo, Florida. The new practice, Coastal Shoulder Surgery & Sports Medicine will be located at 8240 118th Ave Ste 350, Largo, FL 33773-5015.
I will be available to continue your current and any future treatment, here in Columbus until March 29, 2024. Following that, and beginning May 2024, I am happy to continue your care, but at the new location in Florida.
That said, it is still important that you continue your appropriate LOCAL Orthopaedic care. Therefore, I would like to help you establish contact with another Orthopaedic Surgeon as soon as possible. The following information is provided to assist you with this:
1. Dr. Jake Dunn, DO of St. Francis Orthopaedic Institute, has agreed to continue treating all of my patients for their Orthopaedic needs, effective immediately. Dr. Dunn is a board-certified Orthopaedic Surgeon and fellowship trained in Sports Medicine at the Andrews Institute for Orthopedics and Sports Medicine and is certain to continue your first-class care. You may reach out to him and/or his Medical Assistant at St. Francis Orthopaedic Institute, 706-320-3208, located at 2300 Manchester Expressway, Suite 101-A, Columbus, GA 31904, main line 706-322-6646.
2. You may also call the Muscogee County Medical Society 706-322-1254 for assistance locating a new Orthopaedic Surgeon.
Once you have chosen a new Orthopaedic Surgeon, please contact your health insurer to verify whether your new surgeon is within your network and contact your primary care physician in order to see if a new referral is necessary to see them.
The enclosed HIPAA-compliant authorization form is necessary to release a copy of your medical records to you or your new physician. Please complete, sign and return as soon as possible and my office will forward a copy of your medical records to you or the physician you designate. I will be available during the next several weeks to promote a smooth transition and continuity of care.
If you have not selected a new physician by the time my practice closes in Columbus, March 29, 2024, you can obtain acute, critical or emergency care by calling Dr. Jake Dunn, DO at St. Francis 706-320-3208 or 706-322-6646 or by visiting the Piedmont Columbus Regional Hospital or St. Francis Hospital emergency departments. After March 29, 2024, please visit www.coastalshoulder.com and contact me at the new location where I will be happy to continue your care, beginning April 15, 2024, or provide you with your medical records as needed.
